Job Description
The Community Health Worker (CHW) will play a key role in the grassroots implementation of health interventions in Backward block. Covering a designated population of up to 5,000 people across 3 villages, the CHW will support disease screening, health education, case detection, treatment facilitation, and community-level reporting for targeted diseases under the project.
Job Responsibilities
- Conduct door-to-door screening for targeted diseases, including leprosy, lymphatic filariasis, Tuberculosis (TB), malaria, scabies, Japanese Encephalitis and Sickle Cell Disease
- Map households and maintain updated screening and health records for the assigned area.
- Deliver health education sessions using IEC materials to promote awareness, early detection, and stigma reduction.
- Facilitate diagnosis (Dx) and treatment (Rx) by linking identified individuals to health facilities and ensuring follow-up.
- Support referral services for patients needing advanced care or further evaluation.
- Maintain daily activity logs and monthly reports, ensuring timely submission to the supervisor.
- Participate in community meetings, training programs, and regular review meetings.
- Liaise with village-level stakeholders and assist in mobilization during health campaigns and outreach clinics.
Education Qualification/Required Skills & Experience
- Minimum of 10th grade (Matriculation) with 0-2 years’ experience.
Requirements:
- Community engagement or public health-related work. Particularly in tribal/rural areas, will be an added advantage.
- Willingness to work in remote village settings and travel by local transport. Empathetic, and respectful towards community beliefs and practices. Ability to maintain basic written records and reporting formats.
