About the Organization

Pallium India is a national registered charitable trust (No. 693/IV/2003) formed in 2003. It is an NGO based out of Trivandrum, Kerala, with the objective of alleviating serious health-related suffering (SHS) of 7-10 million people in India. We strive to demonstrate quality palliative care, to educate professionals and the public, and facilitate development of palliative care services across India.

About the Fellowship

The Global Fellowship in Palliative Medicine organized by Pallium India, Trivandrum Institute of Palliative Sciences (TIPS), is aimed at creating a pool of physicians who can provide advanced palliative care to people with serious health-related suffering and their families. World Health Organization (WHO), in its fact sheet of 2020, mentions that the training for healthcare professionals in palliative care is often limited or non-existent. The situation in Low and Middle Income Countries (LMICs) is often worse. The short courses available in palliative care may not equip them to deal with complex needs of the patients and families. Many physicians who want to pursue a long-term course in Palliative Care are unable to do so primarily due to the unavailability of such courses. Even when courses are available, taking leave from their current work can significantly impact them. However, they can participate in online courses. One shortcoming of online courses is the lack of clinical attachment, where theory is put into practice. To address this, we have designed a hybrid course with most of the theoretical components delivered online and one month of hands-on training. This course is offered at an affordable cost.

It is now globally accepted that palliative care addresses serious health-related suffering. Universal Health Coverage (UHC) requires, in addition to preventive, promotive, curative, and rehabilitative measures, the inclusion of palliative care. The World Health Assembly resolution 67 of 2014 calls for integration of palliative care into all healthcare at all levels (primary, secondary, and tertiary) across the continuum of care (from the beginning of the suffering to the end).

All the guidelines and declarations in effect, seem to be mostly confined to high income countries. Low and middle income countries (LMICs) which house 84% global population have negligible access to palliative care. The elements of suffering in LMICs would be significantly different from those in higher income countries. The proposed solutions must also consider the cultural and economic realities of LMICs.

How can we change this dismal scenario? As recommended by the World Health Organization, this demands concurrent action on 3 measures:

• Policy and implementation
• Access to essential medicines, (particularly opioids) and
• Education.

Without professional education any policy or legislative change will be ineffective. This is the context in which we are initiating a global fellowship in palliative medicine—to build a network of empowered health care professionals who can address problems and solutions through the lens of low- and middleincome countries (LMICs).

We aim to achieve the following objectives:

• To provide an overview of the practice of palliative care globally with special focus on LMICs.
• To facilitate research and quality improvement programs in palliative care
• To impart attitude, knowledge and skills for practicing palliative care in both cancer and noncancer illness.
• To ensure an understanding of the ground realities and elements of suffering experienced by individuals with serious health-related illness as well as potential solutions involving community participation.

Eligibility

• Any doctor with an MBBS or BDS degree and corresponding medical or dental council registration is eligible to apply for this course. Doctors with disabilities are encouraged to apply. We reserve one seat and will provide reasonable accommodations to participate meaningfully in the program.
• (In accordance with the rules of the Dental Council of India (DCI), the holder of a BDS degree will be eligible to practice palliative care in conditions associated with the oral and maxillofacial area. Under the NDPS rules of 2015, BDS with training in pain management is adequate qualification to be in-charge of a Recognized Medical Institution (RMI) to stock and dispense Essential Narcotic Drugs (END) including morphine, methadone and fentanyl).
