Implementing Agency for CVD Patient Engagement Project – Pilot Phase

Heart Health India Foundation (HHIF) has partnered with the National Health Mission, Madhya Pradesh (NHM-MP) to develop first of its kind heart patient engagement program in the state. HHIF now invites proposals from qualified agencies to implement a pilot project in the Bhopal district. This initiative focuses on establishing Patient Support Groups (PSGs) for heart attack patients and their families, providing them with essential support, education, and expert-led sessions to enhance their recovery journey and overall well-being.

About the Project

The partnership between NHM-MP and HHIF aims to address the unique challenges faced by heart attack survivors and their families. Through structured patient engagement, the project will create a supportive ecosystem to guide patients through recovery, lifestyle adjustments, and emotional resilience, fostering a network for long-term health and empowerment.

Project Objectives

  1. Empower Patients and Families: Equip heart attack patients and their families with knowledge and tools for effective post-attack recovery.
  2. Create a Support Network: Establish peer-led groups to share experiences, encourage motivation, and reduce emotional isolation.
  3. Provide Multidisciplinary Guidance: Offer access to cardiologists, psychologists, physiotherapists, and nutritionists for holistic recovery.
  4. Build a Digital Engagement Platform: Facilitate continuous support and interaction through online groups for ongoing discussions and resources.

Pilot Phase Details

The pilot phase will run for six months in the Bhopal district, with monthly PSG meetings held at district hospitals. Each meeting will be a blend of educational sessions, expert guidance, and peer-to-peer engagement.

Scope of Work

The implementing agency will:

  1. Patient Outreach and Data Management:
    • Collect patient and family data from hospital records and community sources.
    • Contact heart attack survivors and their families to invite them to PSG meetings.
  2. Event Organization:
    • Organize monthly PSG meetings at district hospitals.
    • Arrange for multidisciplinary experts (e.g., cardiologists, psychologists, physiotherapists, nutritionists) to conduct sessions.
    • Provide logistical support for patients and families.
    • Record sessions (video documentation).
    • Prepare and submit detailed event reports.
  3. Digital Engagement:
    • Create and moderate WhatsApp groups for heart attack patients and their families.
    • Encourage active participation, share resources, and address queries in these groups.
  4. Progress Reporting:
    • Submit monthly progress reports summarizing activities, attendance, engagement metrics, and outcomes.

  • Enhanced patient and family knowledge on heart health, recovery, and emotional well-being.

Proposal Requirements

Interested agencies should submit a proposal that includes:

  1. Capability Statement: Highlight your experience with similar projects, particularly patient engagement and health-related initiatives.
  2. Team Expertise: Detail the relevant qualifications and roles of team members.
  3. Approach and Methodology: Explain your strategy for patient engagement, meeting organization, and digital support.
  4. Execution Plan: Provide a timeline and detailed roadmap for the six-month pilot.
  5. Financial Proposal: Include a comprehensive budget for implementing the project.
