About the Organization

The Directorate for the Rehabilitation of the Disabled was established by bifurcating the Directorate of Social Welfare in 1992 to deal exclusively with the Welfare of the Disabled.  Subsequently, the Directorate was upgraded as the office of the State Commissioner for the Disabled in 1999 as per the provisions in the Persons With Disability Act, 1995.

The Government of Tamil Nadu through various policies and initiatives have extended full support to the Differently Abled Persons in their pursuit of full and equal involvement in every aspect of society. Various schemes have been announced by the Government to make sure that the differently abled persons are in equal status to the other people.

The State Policy for the Differently Abled Persons has been issued by the Government in 1994 taking into account of the latest developments in Science and Technology. This policy also provides for a mechanism for promotion and protection of the rights of Differently Abled Persons and methods for ensuring equal opportunities for their full participation in social life.

History is replete with instances of differently abled persons exhibiting exceptional and extraordinary skills.  It is the responsibility of everybody in a civil society to play a vital role in creating an inclusive society where differently abled persons have equal opportunities and full participation in the growth and development of the country. A change of mind set and an approach with human touch towards the challenges faced by differently abled persons are the aims of this Government so that they lead a life of dignity and honour.

About the Proposal

More than 26.8 million people in India and 1.18 million in Tamil Nadu live with some form of disability (Census 2011). The proportion of Differently-Abled Persons (DAPs) was about 1.63% in Tamil Nadu compared to the National average of 2.21% as per Census 2011; however, estimation of the prevalence of disability remains a key challenge both for India and Tamil Nadu due to paucity of prevalence data.

The State Government of Tamil Nadu, under the Department for the Welfare of Differently-Abled Persons (DfWDAP) and the Commissionerate for the Welfare of Differently -Abled Persons (CWDA) through the District Differently-Abled Welfare Office (DDAWO) in every district is implementing a range of programs and services for Differently - Abled Persons. The State is funding various schemes and programs far ahead of other State Governments in making enabling provisions for Differently - Abled Persons. Despite various initiatives taken up by the Government, a sizable proportion of the Differently - Abled Persons population, especially in rural areas, do not have accessible care and rehabilitation services as the outreach and nature of the last mile delivery is fragmented.

To address these gaps and strengthen the reach of last-mile service to Differently- Abled Persons, the Government of Tamil Nadu, through the DfWDAP, is implementing the “TN RIGHTS Project” funded by the World Bank. The project aims to strengthen the State of Tamil Nadu's social protection systems and capability to promote Inclusion, Accessibility, and Opportunities for Differently-Abled persons.

The focus would be improving access and quality of care and rehabilitation services for Differently- Abled Persons by setting up a comprehensive system for social care services throughout the State. This will be done by supporting two major strategic priorities – first, improving the last mile service delivery through family and community based care and rehabilitation services, and second, strengthening the institution-based care and rehabilitation services through a One Stop Social Care Service Centre (OSC) in a public-private partnership approach.

To ensure seamless availability and accessibility to comprehensive social care and services for Differently-Abled - Persons, the project will create a Hub and Spoke model at four tiers (see graph below). The family and community will be the first level of service delivery, where the project with the Community Rehabilitation Workers' (CRWs) support will ensure the delivery of relevant care and rehabilitation services facilitated by a case management approach. The next level of service delivery will be the Neighborhood Centres (NC) set up at the cluster of villages/wards that will act as community-level platforms / safe spaces for community engagement activities, delivery of life skills education, parents/caregiver training, essential therapy services (periodic), access to other social protection services, among others. The third level of service delivery will be the Block level One Stop Social Care Service Centres (Block-OSCs) / Early Intervention Centres (EICs) that will provide centre-based basic care and rehabilitation services to Differently - Abled Persons as well as coordinate and supervise the family and community-based service delivery through the network of CRWs. The fourth and final level of the social care service delivery model will be the Sub-divisional level Integrated One-stop Social Care Centres (OSCs) that will act as the Hub for the respective sub-division. The Subdivisional OSC set up in the District Headquarters will act as the District level Hub for the social care service delivery. These OSCs will provide Differently - Abled Persons with specialised centre-based care and rehabilitation services and coordinate and supervise all the social care services within their catchment area. CWDA will establish the OSC in every sub division, while the remaining three tiers of service delivery will come under these terms of reference.

The Family and Community-Based Care and Rehabilitation Services will be delivered through Community-Based Rehabilitation (CBR) to improve access to quality care and rehabilitation services for Differently - Abled Persons closer to them. CBR is a community development strategy that aims to enhance the quality of life for Differently - Abled Persons/ their families and ensure their inclusion and participation of Differently - Abled Persons in the community. This will include individual-centric planning with the support of family and community, a grass-roots approach to providing care and services, mobilising local capacity and resources, and building on community strengths and structures with the primary purpose of making available comprehensive care and services made available closer to the extent possible to Differently - Abled Persons and their parents/caregivers. This will be done by keeping the Differently - Abled Persons/families as the central focus for their social care services through adopting a case management tool with individual-centric needs assessment and care plans, community-based care, and rehabilitation services led by a cadre of CRW.

To provide the above proposed CBR activities of family and community-based care and rehabilitation services at every Blocks/Municipalities/Corporation Zones level in Tiruppur District, TN RIGHTS intends to engage a qualified and experienced organisation/firm, from now on called a “Community Service Provider (CSP)”. The CSP will be expected to deliver CBR services to Differently - Abled Persons and their parents/caregivers by establishing the required institutional structure and engaging a dedicated team of specialists. The project will provide administrative support, monitoring, and supervision through its District and Block-level administrative units led by the block coordinator.

Objective:

This assignment aims to engage a CSP to set up and manage the institutional structure and engage qualified personnel to deliver care and rehabilitation services to Differently - Abled Persons through CBR in family and community settings. In particular, the CSP, on behalf of the project, will be responsible for the following:

  • Community mobilisation and communication.
  • Individual household-level case management.
  • Family and community-based care and rehabilitation service delivery.
  • Engagement and training of the Community Rehabilitation Workers (CRWs) and Community Facilitators (CFs).
  • CBR services delivered by trained CRWs.
  • Enhancing capabilities of caregivers/parents through training and handholding support.
  • Imparting life skills education to Differently - Abled Persons using community based service delivery.
  • Using technology to link Differently-Abled persons with professional care and rehabilitation services.
  • Establish Block OSC in every Block / Municipality / Corporation Zone of the District and NC at a cluster of villages.
  • Coordination and facilitation with other agencies/service providers for smooth delivery of care and rehabilitation services for Differently - Abled Persons and
  • Other project-related activities, including facilitating and monitoring services provided by various service providers engaged in the project.

Scope of Work:

The broad scope of engaging CSP is providing family and community-based care and rehabilitation. This will include delivering individual and household-level care and rehabilitation services through a case management approach to the Differently - Abled Persons and their families in the Tiruppur District. The CSP will be expected to deliver the CBR services. Detailed operational guidelines will be given by the project separately during the implementation. The responsibilities of the CSP will comprise, but not be limited, to the following:

  • Delivering package of Family and Community Care Services

The CBR activities and the scope of work for delivering a package of family and community care services by the CSP are as follows:

  • Community mobilisation and communication
  • Household-level case management services
  • Engagement and training of Community level front line cadre for family and community-based services
  • Enhancing capabilities of caregivers/parents through training and handholding support
  • Imparting life skills education to Differently - Abled Persons using community-based service deliveries
  • Using technology for linking Differently-Abled persons with professional care and rehabilitation services
  • Establishing Neighborhood Centre

One Neighborhood Centre will be established for every 10 – 15 Village Panchayat/ Wards1 and will act as a safe space for the Differently - Abled Persons, caregivers and their families. The CSP will be responsible for undertaking the mapping exercise of each block assigned to them and submitting an inception report to CWDA proposing the number of NCs based on the terrain distribution, hard-to-reach places, distribution of Differently - Abled Persons, etc. The department will vet the inception report. The department will make space available for the NCs; wherever such provision cannot be made, the NCs may be established in rented space by the CSP as per the cost norms issued by the project separately. This includes a small administrative hall of 200 sq. ft and a large hall for providing training/ rehabilitation with the size of 500 sq. ft with basic amenities and fulfilling the Environmental and Social Management Framework checklist. CF in every Neighborhood Centre to deliver the below package of services to Differently - Abled Persons.

The services to be delivered at the Neighborhood Centre by CF include:

  • Basic rehabilitation and group therapy services.
  • Guidance and access to citizen services.
  • Mobilise caregivers to create peer group support.
  • Impart life skill education to Differently - Abled Persons/ caregivers assisted by the block service provider team.
  • Create a platform for community engagement of Differently - Abled Persons/ caregivers and associations to identify common services required.
  • Facilitate access to beneficiary-oriented schemes and programs by providing information and basic support in application, etc.
  • Establishing Block level OSC (Block-OSC)

A Block-OSC will be established in every Block / Municipality / Corporation Zone. The project will appoint the administration team, comprising a block coordinator for each block. The CSP and the DPIU will map and identify accessible locations for establishing the Block OSC for each block assigned to them. Efforts will be made to identify Government premises for establishing the Block-OSC, preferably co-located within Health & Wellness Centres or close to such facilities for better coordination. However, suppose CWDA is unable to make Government premises available for these centres. In that case, the Block-OSC will be established in a rented premise identified by the CSP following the required criteria. The rent for these centres will be reimbursed to the CSP per the cost norms issued by the project separately. About 2000 sq. ft of space will be required for each Block-OSC, including an administrative unit, therapy unit, and two special education units with basic amenities and fulfilling the Environmental and Social Management Framework checklist.

The broad services to be delivered at the Block-OSC

  • Awareness generation, public education and mobilisation of target groups.
  • Case assessment and individual care plans.
  • Psycho-social and emotional support through counselling services.
  • Assessment, determination and delivery of the relevant therapy and other rehabilitation services.
  • Early identification and intervention in a life-cycle approach.
  • Specialised services, including providing assistive devices in coordination with the DDAWO.
  • The E-seva maiyam desk will impart Information on other social protection schemes and access to its services /benefits.
  • Tele - Rehabilitation services through a team of pre-identified rehabilitation specialists with support from the CRWs.
  • Need-based Referral Services to specialised facilities.
